Student ID Card Loss Report and Reissuance

Important Notice:
Once a Student ID card is reported as lost, it cannot be reactivated.
Please make sure you have carefully confirmed before submitting the application.

Student ID Card Reissuance Procedure

I. Online Application

  1. Log in to NTNU Information Portal, then go to: Applications → Information Services < Student ID Card Loss Report and Reissuance System>, and enter the“Reissue a New Card”.
  2. Complete the required information and submit the application.
•Important Notes: Once the application is submitted, the EasyCard (transportation) function of the Student ID card will be deactivated immediately and the loss report cannot be canceled.
•If the original card is found before a new card is produced and the EasyCard function is not required (the card is only needed for campus access or student identification), students may apply to cancel card production and restore campus access privileges of the original card at:
  • the Academic Affairs Office Service Counter (1st Floor, Administration Building I, Heping Campus I), or
  • the Gongguan Academic Affairs Office.
•If the new card has already been produced, cancellation of reissuance and restoration of the original card’s campus access privileges are not permitted.
•If a photo replacement is required, please apply for photo replacement first. After confirming that the photo has been updated, proceed with payment.
•Card collection locations:
Main Campus (Heping):
Academic Affairs Office Service Counter (Registration Division),
1st Floor, Administration Building I, Heping Campus (Campus Map H101)
Gongguan Campus:
Academic Affairs, Gongguan Campus
1st Floor, General Building, Gongguan Campus (Campus Map G102)

II. Payment of Reissuance Fee (NTD 200)

* Exchange students and visiting students must make payment at the service counter.
Payment Methods:
  1. Credit Card: Select credit card payment in the system (currently unavailable due to system maintenance).
  2. EasyCard Payment: Pay using an EasyCard at a Campus E-card Kiosk and obtain a receipt. Return to the Student ID Card Reissuance System and enter the application number shown on the receipt.
    Campus E-card Kiosk Locations:
    •Administration Building I, 1st Floor (outside the Registration Division), Heping Campus I (Campus Map H101)
    •Library, 1st Floor, Heping Campus II (Campus Map H202)
    •Residence Hall A, 1st Floor Common Area (Campus Map H127)
    •General Building, 1st Floor, Gongguan Campus (Campus Map G102)
  3. Cash Payment:
    •Academic Affairs Office Service Counter (Registration Division),1st Floor, Administration Building I, Heping Campus I (Campus Map H101)
    •Academic Affairs, Gongguan Campus, 1st Floor, General Building, Gongguan Campus (Campus Map G102)
* If payment is not completed within one month after submitting the application, the system will automatically cancel the application.

III. Application Status

After payment is completed, the system status will display “Processing”, indicating that the reissuance application has been successfully submitted.
When the status changes to “Card Produced”, the Student ID card is ready for collection (usually on the same day or the next business day). Notification will be sent via email.

IV. Card Collection

The applicant must present one of the following at the selected campus:
Original identification document, or Digital Student ID displayed inNTNU app, or Original payment receipt

*If the applicant is unable to collect the card in person, an authorized representative may do so by presenting:
A letter of authorization, and
The original identification document of the authorized representative

Loss Report Only (No Reissuance)

For reporting a lost Student ID card without applying for reissuance:
  1. Log in to NTNU Information Portal, then go to: Applications → Information Services < Student ID Card Loss Report and Reissuance System>, and enter the“Report a Lost Card”.
  2. Complete the required information and submit the application.
* Please note that once submitted, the EasyCard function will be deactivated immediately and the loss report cannot be canceled.

EasyCard Balance Refund Information

The remaining balance on a reported EasyCard will be refunded by the EasyCard Corporation via bank transfer to your designated bank account or by mailing a refund notice to your registered address.
The processing time is approximately 20 working days.

The EasyCard Corporation will deduct the following fees from the remaining balance:
Loss report / refund handling fee: NTD 20
Bank transfer fee: NTD 0 (currently waived), or Registered mail postage: NTD 28
